You can select copay levels, formularies, deductibles, and cost-management programs from Millennium. Plans cover outpatient prescription drugs* and prescribed injectable insulin.
Premiums are guaranteed for 12 months. Special participation requirements and rules apply to groups of 150 or more employees.
